I remembered the third question, "How is my libido?". I had started to write that I don't have much of a libido since I'm not married or in a relationship and realized that seemed male oriented too. That is when I looked at the top of the form and saw the word "Male". As I said, I was very happy that when I got into the exam room I was asked many of the questions I had already answered on the form. Thank goodness.
